Need my book to be created for print and E-Book style

I already have a book completed. I scanned all the pages and need it to be done all over again for me to have it loaded for Amazon print and E-Book. It is 250 pages and its pretty simple task to just copy everything and put it in the correct form in order to have it ready to be sent to Amazon for print and e-book formats ready to sell online. I do need a new book cover and of course, someone to create the copyright page and everything else that I may need. As I stated, I already have the book pages written and ready to be copied put on a better book cover from what I have.
