Build Automated Reporting Platform with CMS Integration

Our company provides onsite injury prevention services, and we're looking to streamline our reporting process. We need a robust platform built to automate how our specialists collect data and generate client reports. Project Goal: Create a system where our providers use digital forms for data entry, the system pulls related data from another software (CMS Wellness), combines everything, and generates standardized monthly/quarterly/annual reports. Technical Approach: We are open to different technical solutions to achieve this. While a fully custom-coded solution using standard web technologies is acceptable, we are also strongly considering proposals utilizing specific low-code/no-code platforms. The platforms we are potentially interested in are: - Microsoft Power Platform (preferred) - Caspio (preferred) - Zoho Creator - Appian If proposing a low-code solution, please specify the platform and detail how it will meet all core requirements below, especially the critical CMS Wellness API integration and the custom reporting logic/output. Core Requirements (Must be met by either custom or low-code solution): Data Input: Build a user-friendly web interface for our providers to complete 6 distinct digital forms. These should use efficient inputs like dropdowns, checkboxes, etc., based on templates we provide. Database/Data Storage: Create or configure a secure backend to store the data entered via these forms. CMS Wellness Integration: This is critical. The system must automatically pull specific data (client details, appointment info, survey results, etc.) from our existing "CMS Wellness" software, likely via its API. Proven ability to handle third-party API integrations is essential. Automated Report Generation: Develop/configure the logic to: Combine data from the new forms and the pulled CMS data. Populate report templates (which we will provide) for monthly, quarterly, and annual reports. Perform calculations based on formulas we provide (e.g., risk levels). Allow providers to add free-text narrative interpretations into designated fields within the generated report before finalizing/exporting (e.g., as a PDF). What We Provide: Detailed templates for the 6 input forms. Detailed templates for the output reports (monthly/quarterly/annual), including layout, data sources, narrative field locations. Specific formulas for necessary calculations. Your focus (whether custom or low-code) will be on successfully configuring/building the database structure, the backend processing logic, the critical CMS Wellness integration, the front-end input forms, and the report generation engine to meet our specific needs based on the provided templates and formulas. Deliverables: Fully functional and tested application/platform. Complete source code (if custom) or export/configuration details (if low-code). Basic user/technical documentation. Who We're Looking For: We're seeking an experienced individual freelancer or a small agency with a strong track record demonstrating capability in EITHER custom development OR one of the specified low-code platforms: For Custom Solutions: Expertise in Full-Stack Web Development (Backend: Python, Node.js, Java, C#, etc.; Frontend: React, Angular, Vue, HTML, CSS, JS), Database Design & SQL. For Low-Code Solutions: Proven expertise developing complex applications using Microsoft Power Platform, Caspio, Zoho Creator, OR Appian. Please specify which platform(s) you are proficient in. Crucially, for EITHER approach, you MUST have: Strong API Integration Skills: Demonstrable experience successfully configuring/integrating with third-party APIs (especially REST/JSON). This is non-negotiable. Please provide specific examples related to the platform you propose or your custom development experience. Database/Data Structuring Skills: Ability to design or configure logical data storage. Experience with Business Applications: Creating data-driven applications, ideally related to reporting, workflow automation, or data processing.
