AI Agent & Automation Setup for Clinic Data Collection and Database Integration

We are seeking an automation expert to build an AI agent for MyBonum, our therapy platform, to collect and update clinic data across Florida. Scope includes creating a dynamic survey form (yes/no, checkboxes, dropdowns), automating email outreach and follow-ups, and integrating responses into our MyBonum database via API or Zapier. Optional: Twilio-based phone or SMS follow-ups. Clinics will update fields like languages spoken, insurance accepted, patient intake status, and service models (mobile/clinic/telehealth).
