Format nonfiction manuscript in Word for Mac

I have a 67,000 word nonfiction manuscript in Word for Mac that I've worked on for more than five years. It's a "memoir" covering my time on Molokai as a community organizer, 1973-75. It is backed up with verifiable publications and accounts of the time, principally newspaper reports, resulting in about 150 end notes. It has 32 chapters, some quite short. I have almost finished writing and would like to have it formatted, with the table of contents connected to the chapters so that I can easily input more text and end notes.
